Option 1: Online Registration

lick on the link below labeled “Register Online” to register online.  If you register on line,
you  may pay by credit card or check.  Due to increased costs with online registration, there is a
$4 processing fee for online registration. Online will save you time and the dreaded walk in
process so we hope you don't mind the small fee.

You can now UPLOAD your child's photograph during the registration process!!!
Follow the instructions in the registration program when prompted to upload a photo. (Photo
is only needed for players age 8 and up)

** Returning players: After logging in with your user id and password, please make sure you 
scroll down and select the 2013 season to start!
  Otherwise, you may get an error message stating
the season is closed **

After registering online you must submit your required documents by mail 
Problems with online registration?  Please call Active Customer Service at 1-877-228-4881.

Items to mail in:

If new to LYSC:

Photo copy of player's birth certificate. Must be issued from the county/state of birth (not from the
hospital of birth).  Passports are acceptable.

For players age 8 and up: 

If you do not elect to upload a picture, you must mail one in as follows:

1" x 1" color headshot of player being registered (no sunglasses or hats please). 
Important: the head/face portion of this photo should occupy at least 1"x1" of the photo space.
 Please do not deviate from these specifications or we will be calling you for a new picture,
 delaying your application. Please PRINT the name of your child on the back of the picture.

*  If paying by check, please make payable to “LYSC” in the amount of $80 (recreation players) &
$90 for Comp. Multiple player discount is $10 less each player ($10 for any subsequent child:
$70 each ($80 if playing Comp). If mailed in after May 15th, please include $25 per player ($105)
or packet will be returned.

Mail the registration form (2 signed copies), birth certificate, photo, and money to:

Failure to return the paperwork will delay the registration process and the player may not
be placed on a team for the season. All paperwork must be received by June 1st to guarantee
a spot on a team.  You will receive reminder e-mails that your packet has
not been received or is incomplete.

DUE TO THE SIZE OF OUR CLUB, WE CAN NO LONGER ACCEPT FRIEND REQUESTS. 

WE WILL DO OUR BEST FOR COACH REQUESTS BUT PLEASE ENSURE THE COACH IS
RETURNING AND IN THE SAME AGE BRACKET AS YOUR CHILD. RETURNING PLAYERS
HAVE PRIORITY ON THE ROSTER.

** INCOMPLETE PACKETS WILL NOT BE ACCEPTED **   

Option 2: Attend one of these Registrations Sign-Up events:.

Dates:

April 6th:    2-5 p.m. at the Community Center
April 27th:  2-5 p.m. at the Community Center

The Community Center is at 1st and Joiner Parkway and will be held in the classrooms past
the office.

Items to bring to Registration

If new to LYSC:

A personal check, cashier's check, or money order (per player) made out to:

To speed up the process, download, fill out the registration form and bring it with you: 
(click here to download form) .  TYPE in the information, print TWO copies and sign both in the
lower right corner.

Please provide at least 2 phone numbers and an e-mail address.

Including an e-mail address is highly recommended, so that you can receive soccer club news
in a timely manner.

Sign the form.

If any items are not provided at the registration night, the packet will not be accepted.  Please
ensure you bring all required items to the walk in dates. 

 
IMPORTANT INFORMATION

Age Groups 
Please refer to the
Age Group Matrix to determine which Age-Group your child will play in.

Late Registration

Late registration begins on May 15th and closes on June 15th.  If possible, you want to register
as soon as possible, prior to May 15th to avoid late fees ($25) and to allow your player to be
placed on a team.  Late registration is a time consuming process, and can result in
complications if the correct age-group team already has a full roster.  We can not guarantee
a spot on a team for any packets received after June 1st and will keep a waiting list.

Bounced Check Policy

A certain number of registration checks that the club receives will be returned for Insufficient
Funds.  In these cases, we cannot accept another personal check.  Two options are available:
1) Cash or 2) Money Order.  In either case, you need to submit the original amount plus $15 to
cover the bank fees that the club is charged.

Refund Policy

Please click here for the form to request a refund.  Deadlines do apply so please ensure you review and mail timely.  All refund requests will have a $5 processing fee.

 

 

Uniforms

Uniforms can be purchased at Extreme Soccer located at 928 Pleasant Grove Blvd # 150 in
Rocklin after August 1st.   Uniform flyer can be downloaded from our site under the
“Download” Section or click here.
